Types of Tooth Whitening

There are two basic options for smile whitening:

In-office tooth whitening:

instant whitening results using light-activated gel to brighten teeth. In-office tooth whitening gives immediate whitening results after treatment. At Patong Smile Dentacl Clinic, we utilize Laser teeth whitening, PHILIPS ZOOM! Advanced Power professional tooth whitening system, LED( New Advance) and Opalescence Boost teeth whitening. We are the only one dental clinic that has many technologies for in-office tooth whitening systems. So that you and dentist can select the technique which is the most suitable for you.

At-home whitening systems:

Each person will receive customized trays and prescription opalescence whitening gel done at home. At-home tooth whitening systems need to be done as directed by your dentist over a period of time to attain the result warranted

Procedure for In-office Tooth Whitening Treatment
  1. First Evaluation and pre-whitening tooth shade selection
    1. the pre-whitening tooth shade is selected from array of tooth-shaped porcelain tabs (each of which is a slightly different color) and compare each tab with their patient’s teeth for post-treatment comparison
    2. professional dental scaling and polishing is usually done before the bleaching treatment is performed, but it is optional if the patient's teeth is already cleaned
  2. Isolate Teeth to be Whitened
    1. A dental dam is placed to isolate teeth and prevent the whitening agent from irritating or damaging soft oral tissues
  3. Place Eye Protection for protection
  4. Apply whitening gel
    1. teeth is dried and the bleaching compound applied
  5. Expose whitening gel to laser light / Zoom light /or LED light
    1. The whitening gel is activated using a light source. This "activation" shortens the needed treatment time and/or makes the whitener more effective
  6. Leave whitening gel for a prescribed time or repeat 4 and 5
  7. Remove whitening gel
    1. the whitening gel and dental dam is washed and suctioned off
  8. Determine Post-bleaching Tooth Shade
    1. after the bleaching treatment has been completed, a shade evaluation is done to see how much whitening is achieved
Recovery Expectations

Some people who undergo in-office whitening may experience some sensitivity in their teeth. This sensitivity should be gone within 1-2 days. Sensitivity from at home whitening will dissipate after a 1-2 day rest between whitening applications.

Post care Instructions for Tooth Whitening (In-Office)
  1. Avoid consuming color-staining foods and drinks such as coffee, tea, berry pie, soy sauce, red wine, mustard or ketchup, coke, etc. as well as smoking for at least one week
  2. Proper oral hygiene care should be carried out to maintain and avoid diminished whitening results
  3. If experiencing some sensitivity in teeth after tooth whitening
    1. Avoid hot, cold or acidic food and beverages
    2. Pain medication be taken as directed as long as there is no medical contradiction based upon your medical history
    3. Use fluoride rinse and toothpaste for sensitivity teeth
  4. If experiencing some sensitivity in teeth for take-home bleaching
    1. Wear the tray for a shorter period of time (for example, two 30-minute sessions vs. two 60-minute sessions)
    2. Stop whitening your teeth for two to three days to allow your teeth to adjust to the whitening process

At-home whitening systems (Home Bleaching)

The dentist molds vinyl tray to fit your teeth and then gives you a prescribed gel containing carbomide peroxide to put in the trays. You go home load the gel in the trays wear the trays in place for six to eight hours a day for 2-3 weeks. When the peroxide begins its work it introduces oxygen into the enamel of your teeth, literally bleaching any staining element present. Only natural or enamel can be whitened, veneer, crowns and filling can not be lightened. Obviously with this the best time is bedtime because there should be little interruption. The gel is a gentle solution and does not damage the tooth structure.

It must always be noted that even if teeth are successfully whitened, retaining habits or exposing your teeth to vulnerable variables such as eating dark fruit or drinking coffee or smoking will only darken your teeth again.

Opalescence® is a fast, safe and effective tooth whitening system. The whitening gel is a thick solution, which is dispensed to patients by a dentist and applied by the patient at home using a clear and inconspicuous custom fitted tray. The unique "scalloped" tray with reservoirs was developed to minimize tray contact and peroxide gel exposure to soft tissues. The Opalescence PF sticky formula does not leach from the tray as did former tooth whitening preparations.

The delicious mint flavored gel is worn overnight, while you sleep for ultimate performance and maximum convenience. The patented formula bleaches in as little as 30 minutes in 1-3 daily treatments or for 1-6 hours daytime (if worn overnight, gel will stay active for 8-10 hours).

The active ingredient in Opalescence®, carbamide peroxide, has been recognized by FDA for many years as an oral antiseptic and is now used for its superior tooth whitening properties. Opalescence® is available in a variety of carbamide peroxide concentrations to whiten even the most difficult stains.

Opalescence PF is recommended for whitening discolored teeth prior to placement of composite, veneers, and/or crowns. Wait approximately two weeks following the bleaching procedure to assure shade has stabilized before matching restorations to bleached enamel. Porcelain and other restorative materials will not whiten. Many times patients with lighter porcelain restorations can benefit from having adjacent darker natural teeth lightened to match the lighter restorations.

The Opalescence formula has shown to be effective approximately 95% of the time for removing some or all of the discoloration and/or stains due to natural, congenital, systemic, pharmalogic, traumatic, etc. factors. Success is most always obtained with fluorosis and to significant degrees with tetracycline staining, depending on the patient compliance and/or duration of treatment.

Age and pregnancy issues.

Bleaching is not recommended in children under the age of 16. This is because the pulp chamber, or nerve of the tooth, is enlarged until this age. Teeth whitening under this condition could irritate the pulp or cause it to become sensitive. Teeth whitening is also not recommended in pregnant or lactating women.

Sensitive teeth and allergies to products.

Individuals with sensitive teeth and gums, receding gums and/or defective restorations should consult with their dentist prior to using a tooth whitening system. Anyone allergic to peroxide (the whitening agent) should not use a bleaching product.

Gum disease, worn enamel, cavities, and exposed roots.

Individuals with gum disease or teeth with worn enamel are generally discouraged from undergoing a tooth whitening procedure. Cavities need to be treated before undergoing any whitening procedure. This is because the whitening solutions penetrate into any existing decay and the inner areas of the tooth, which can cause sensitivity. Also, whitening procedures will not work on exposed tooth roots because roots do not have an enamel layer.

Fillings, crowns and other restorations.

Tooth-colored fillings and resin composite materials used in dental restorations (crowns, veneers, bonding, bridges) do not whiten. Therefore, using a whitening agent on teeth that do and do not contain restorations will results in uneven whitening-in this case, making the teeth without restorations appear lighter than those with restorations. Any whitening procedure should be done prior to the placement of composite fillings, bonding, veneers, crowns, dentures, or porcelain restorations in order to best match the degree of whitening to your new tooth color. A minimum of 2 weeks following a whitening procedure should be allowed before crowns, bondings, or veneers are completed. This will allow enough time for the enamel to remineralize and optimize the bonding strength. Tooth-colored fillings will need to be replaced after the bleaching process is complete. Individuals with numerous restorations that would result in uneven whitening may be better off considering bonding, veneers or crowns rather than a tooth whitening system. Ask your dentist what strategy may be best for you.

Unrealistic expectations.

Individuals who expect their teeth to be a new "blinding white" may be disappointed with their results. Smokers need to be aware that their results will be limited unless they refrain from continued smoking, particularly during the bleaching process. A healthy guide as to a reasonable degree of whiteness to achieve with a whitening process that would give a natural appearance to a person's teeth is a slightly whiter color than the whites of your eyes.

Darkly stained teeth.

Yellow-ish teeth respond well to bleaching, brownish-colored teeth respond less well, and grayish-hue or purple-stained teeth may not respond well to bleaching at all. Blue-gray staining caused by tetracycline is more difficult to lighten and may require up to 6 months of home treatments or several in-office appointments to successfully lighten. Teeth that have dark stains may be better candidates for another lightening option, such as veneers , bonding , or crowns . Your dentist can discuss the options best suited for your situation.

Laser whitening

Laser procedure

With laser bleaching a dentist applies a peroxide solution to your teeth and then uses light or a laser to accelerate the whitening process. It takes approximately 1 hour to complete the procedure. The process involves no additional at home treatment.

Laser tooth whitening has a satisfaction rate of about 93%. The long-term results depend primarily on the individual. Certain habits, such as using tobacco products, drinking coffee, tea, dark soft drinks, and infrequent visits to the dentist may lessen results. As long as you receive regular professional care and use professional after care products (whitening paste, etc.) you should be able to maintain results for a period of 1-2 years.

Laser tooth whitening is safe and will not soften enamel or existing fillings. The main ingredient in the gel that's used during the process is hydrogen peroxide. Dentists have used hydrogen peroxide solutions to whiten teeth for years. The percentage of hydrogen peroxide used in laser tooth whitening is 15%. This mixture is considered safe for all gum types and does not produce sensitivity.

Whitening will not correct certain forms of discoloration. For these cases, a dentist may recommend veneers instead which cover the entire tooth. Teeth with prosthetics, bonding, tooth-colored fillings will not bleach. Individuals with yellow tones to their teeth respond best. The degree of whiteness will vary from individual to individual depending on the condition of the teeth, nature of the stain and individual reaction to the whitening agent.

Laser Teeth Whitening Faq
So far, all studies have concluded laser teeth whitening to be completely safe and found that no damage was done to the tooth enamel.
You can expect these results to last for 2-3 years. But up to your habits. If you drinking tea, coffee, red wine, or ketchup, or from smoking, should to touch-up with home whitening gel because your teeth will be faster dark.
Our laser teeth whitening do not heat the tooth. Those with very sensitive teeth may experience some minor discomfort, but this should go away a few hours after the surgery.
You can expect to improve your smile 5 to 10 shades. These results are far better than over-the-counter gels and toothpastes, and the procedure only takes about an hour.
Unfortunately, it is impossible to whiten crowns, caps, and veneers with laser teeth whitening. However, the procedure may remove stains that may have developed over the years, your crowns, caps, and veneers will look whiter.
Many of the reasons for teeth discoloration are widely known. Smoking or drinking dark beverages such as wine or coffee over long periods of time are common culprits. Will it hurt and are there any side effects ?

LED Whitening System : How?
The initiation occurs through the application of light which emits monochromatic. Light at the specific wave length absorbed by catalyst molecules only and by accelerating the release of activated bleaching agent.

The chemical reaction.
Hydrogen peroxide breaks down to water and free oxygen radical. The free radicals chemically reduce larger organic-pigmented molecules in the enamel matrix into smaller, less pigmented constituents by rapid oxidation. These compounds have double bonds and long carbon chains that are subsequently reduced to smaller carbon chains and hydroxyl groups, which eliminate discoloration.

Hydrogen peroxide, the most accepted active ingredient for in-office bleaching, does not decompose readily which is why traditional bleaching procedures require lengthy visits.

Is the procedure safe?

Yes, LED whitening is a very safe procedure.
It does not heat the tooth thus no harm is done to the tooth vitality. The LED whitening system has been specifically developed and tested to ensure that no harm is done to either enamel or dentine and that the pH level is neutral and will not produce any acidic effect associated with some other whitening products.

Will the procedure hurt?

No. this is a very simple procedure where no anesthesia is needed as no pain is experienced. However some patients may experience minor sensitivity during or following the procedure. This sensitivity lasts no more than a couple of hours and it can be resolved by taking an analgesic.

Is the treatment suitable for everyone?
